Web forms that put every lead straight into your pipeline

Build a form, drop it on your site with one tag, and let each enquiry reach a salesperson on its own.

A contact form that emails someone is where good leads go to wait. With a CRM Dost web form, each submission becomes a lead in your pipeline, already assigned to someone on your team.

Build it without code

  • Drag fields into order, and add validation to each one.
  • Use dropdowns, radio buttons and checkboxes, and show a field only when an earlier answer calls for it.
  • Capture the campaign tags, page and referrer behind each submission without asking the visitor.
  • Add a consent field, then show a thank-you message or send people to a page of your choice.
  • Pick a light or dark theme and check it in the live preview.

Put it on your website

Paste one placeholder and one script tag into your site. When you change the form later, your website picks up the change within about five minutes — no need to touch the site again. If you use an automation tool such as Zapier or Make, a separate API mode lets it send leads in directly, with a log of every request.

Every lead lands with someone

Choose which team members a form feeds, and each new lead is assigned to one of them at random, so enquiries are shared out without anyone having to hand them round.

Each submission needs a phone number or an email. If the same person submits again, it is added to their existing lead’s history instead of creating a duplicate.

Files, when you need them

Forms can accept uploads such as a floor plan or a document, in JPG, PNG, WEBP or PDF, up to 5 MB by default.

Keep forms short. Ask for a name and one way to get in touch, and let your salesperson ask the rest on the first call.
